Wauchope Quality Healthcare is now closed permanently. Correspondence regarding patients will need to be forwarded to their new GP practice. Telephone and email addresses will no longer be used after the 30/11/2022.
For patients wishing to obtain their medical records:
Once you have selected your new GP, your new GP will ask you to complete a request form for permission to transfer of your medical records to the new surgery.
Please note that all requests will need to be sent to PO Box 637, Wauchope NSW 2446.
